To schedule an appointment, please call our office at (801) 619-9000 or use our online Request an Appointment link below.
If at any time you are unable to keep your appointment, please notify us at least 24 to 48 business hours in advance. A cancellation fee may be charged. We will be glad to reschedule your appointment at a more favorable time. We greatly appreciate your time and consideration and look forward to seeing you.
