You may bring one visitor age 16 or older to your appointment. No children are allowed at appointments.   1. Check your text messages - they are sent 7 days prior to your appointment and again 24 hours prior.
  2. Check the Prep Info/FAQs page in the main menu above. Choose the procedure prep you were instructed to use.

Post-Procedure Symptoms

Call 801-619-1115 and ask for a nurse or the after-hours On-Call Physician. If symptoms are severe, please go to the nearest Emergency Room.

Registration Invitation

Invitations are sent via email and text message 4 days before your appointment and again, at 24 hours prior if you have not registered. Contact our office at 801-619-1115 if you did not receive your invitation.

We want every patient to receive excellent care at our facilities. If you have a concern or problem with the care you received, please contact the clinic where you were seen and ask for the clinic manager. They are happy to promptly address and find solutions to any patient concerns.
