What insurances panels are the physicians covered under?

Our physicians are providers for all major insurance companies in Utah, as well as, over 200 other insurance panels.

Can my spouse pick up my medical records or call for my test results?

Anyone can have access to your records if a Designated Representative Form (DPR) has been completed by the patient. The person designated must be able to prove their identity to the staff.

How do I obtain a copy of my medical records?

Download the Authorization for Release of Medical Information form.

Complete the form and mail or fax to 801.619.9001.

Why did I receive two statements from you for my procedure?

If you have a procedure performed at Granite Peaks Endoscopy Center or Alta View Hospital you will receive two statements.

Why did I receive a bill from Caris Pathology Group or Intermountain Health?

When biopsies are taken during a procedure, it must be sent off to be read by a pathologist. If this happens, then you will receive a separate bill for the reading of the biopsy. If you have any questions about that bill, please call the appropriate lab and they will be able to answer any of your billing questions.

What is the cancellation policy?

In an effort to provide the best quality of care to our patients and to staff our endoscopy center appropriately, we require 24 hours notification for any appointment cancellations. Failure to do so may result in a cancellation fee of $100.

Is there a processing fee for records?

No, there is not a charge for copying and mailing records pertaining to patient care. If records are requested for disability insurance, then there is processing fee of $15.
