Physician referrals

In order to make the referral process as easy and convenient as possible, we offer several methods below from which to choose.

Telephone referrals

Our scheduling personnel will be happy to make appointment requests via telephone. Just call our office at 801.619.9000 with the patients name, contact and insurance information, and the desired service to be performed, (office consultation or procedure). Faxing (801.619.9001) pertinent patient records to us at the time that the appointment is made would be greatly appreciated.

Fax referrals

You may also fax over your request by printing the form below. Please complete it in its entirety and fax along with patient records to our office. We will contact patient and schedule appointment and communicate with you when the patient is scheduled.
